Informed Consent Form for
Psychological Testing
The signature below indicates my awareness and understanding of the conditions under which I am being required to complete the Minnesota Multiphasic Personality Inventory-2 (MMPI-2).I understand that:
1. As stated in the admissions policy approved by the Murray State University Board of Regents for the baccalaureate Social Work Program, the results of the MMPI-2 alone will not be used to deny me admission to the program. However, students may be required to seek counseling to resolve issues identified by psychological staff who are charged with interpreting my results.
2. The MMPI-2 will be used only for counseling prior to admissions to the Murray State University Social Work Program. All counseling will be provided by appropriately trained psychological staff with a minimum of master's level training in counseling, social work, or clinical psychology.
3. An interpretation of my results on the MMPI-2 will be made available to me if I request it.
4. Information provided by me on the MMPI-2 will be confidential, with only the following individuals having access to the results:
a. myself
b. faculty of the Murray State University Social Work Program
c. psychological staff providing interpretation and counseling services.
