This form is to be used in determining the student's field performance at two points: at midterm and at the conclusion of the semester.
To encourage the student's self-assessment, this instrument is designed to allow students to rate themselves on each area of performance. Students should be allowed to complete the form before the supervisor makes his or her assessment. This will provide a rich source of information for use during the midterm and the final evaluation conferences.
Directions: Circle the appropriate number on the scale in each section. If any of the comments at the level you choose do not fit, feel free to cross them out. Likewise, if a comment in one of the other boxes fits, but overall the student is not entirely at that other level of performance, feel free to underline the other comments to indicate their appropriateness. The scale was designed to show the following levels of performance in each of the twenty-one areas:
0 = failure 1 = marginal abilities; unsatisfactory performance 2 = satisfactory performance 3 = above average performance 4 = exceptional performance
