We are deeply grateful to our many colleagues, including some of the country's most prominent health policy researchers, who made this book possible. The twenty authors who contributed chapters addressed some of the most pressing policy challenges facing our nation and graciously tolerated our editorial prodding.

This volume was supported by The Robert Wood Johnson Foundation through its national program, Investigator Awards in Health Policy Research, that seeks to broaden understanding of health policy challenges and choices. Risa LavizzoMourey, the president and chief executive officer of The Robert Wood Johnson Foundation, and Steve Schroeder, the foundation's past president and CEO, have been especially helpful over the years.
