William and Marie: Sibling
Incest, Sexual Masochism, and
Sexual Sadism
(DSM-III-R-302.83;
DSM-III-R-302.84)

William and Marie experienced abnormal sexual inclinations and
behaviors. Brother and sister, they felt strong sexual arousal for
each other but no one else. They had sexualized their relationship
when they were children. William was seven years older than his
sister, Marie, and had begun to initiate her into sex play when she
was five. He knew he was doing something "wrong," but she had
had no idea that she was misbehaving. She thought it was just a
game, a secret game that they did in private and never talked
about. It wasn't until she was nine that she realized she was "bad"
for "playing nasty" with William. Marie enjoyed the feelings she
had during their sex play but began to think of herself as "bad" be-
cause of the secrecy involved. Sex was bad, and she was bad.

William also felt guilty, doubly so because he had taught his
little sister to "play nasty" with him. He had been initiated into sex
play by a teenage babysitter when he was nine. The babysitter had
fondled him and taught him how to masturbate himself. Unlike his
sister Marie, at the outset he knew it was wrong, but he enjoyed the
feelings and told no one. Later he repeated what he had learned
and eroticized his sister's response, giving her the same inclination.

Incidents like these are common enough and do not always lead
to psychosexual disorder. Usually children go on to have sexual ex-
periences with appropriate partners and the guilt recedes into the
past, along with the memories of the incidents themselves. This did
