The manuscript has been written and rewritten at five universities on four continents. The outline was developed at Montana State University. The first draft was written while on a Fulbright at the University of Bucharest 1994-95. It was revised the next year in Pakistan, while I was at the University of Agriculture at Faisalabad. The third draft was edited at the University of New England at Armidale, New South Wales. The final draft was prepared at the University of New Mexico, which provided an adjunct professorship and a quiet office. Special thanks are due to Professor Philip May, director of the Center on Alcoholism, Substance Abuse, and Addictions (CASAA), and to Professor Richard Coughlin, chairman of the Department of Sociology. Following that, the copy editorial assistance of Deborah Whitford and Nancy Lucas was superb. The research was initiated without funding. Fortunately, the U. S. Department of Agriculture (USDA) sporadically supported the research from the mid-1970s until the late 1980s.
