I am grateful to have received an Evangelical Scholars’ Research Fellowship from the Pew Charitable Trust for the academic year 1997–98 which allowed me to complete the first full manuscript of this book. I particularly want to thank Nathan O. Hatch, the director of the Evangelical Scholars’ Program, and Michael S. Hamilton, its coordinator, for their help and support. At the summer conference of research fellows in 1998 I was able to present my work in progress and to receive valuable criticism and suggestions.
