The focus of this book is a series of detailed descriptions of clinical work with children. The contributors seek to understand their patients' problems by drawing upon a fundamentally psychoanalytic orientation. Their approach is influenced by recent advances in the understanding of infant development.

This book shows the child analyst atwork; the clinical chapters give detailed accounts of the interaction between analyst and child. In presenting these case studies, we have been mindful of the need to preserve confidentiality.Patients, both adults and children, have the right for their privacy tobe respected, and all the families in this book have beendisguised.

A particular difficulty regarding confidentiality arises inthe case of work with children, as concurrent work with the parentsis usually an essential part of the treatment approach. But topublish details of work with parents alongside accounts of a child'streatment makes it more difficult to preserve patients' anonymity.We have therefore omitted any detailed account of work withparents, but we wish to acknowledge the commitment and courage of the parents of the children in this book, most of whom not onlysupported their child's treatment, but were also prepared to facetheir own difficulties where necessary.

It is not possible to disguise clinical material in such a waythat it will be unrecognizable to the patient concerned. We shareour work with others so that mutual learning may take place andour ability to help troubled children grow. Patrick Casement (1985)has thoughtfully explored the issues of confidentiality andpublication, and we conclude with his words: "Ifpatients...have gained as much from the clinical encounter as I have, I hope they may beglad that I have considered it to be worth sharing some of thiswith others" (p. 226).
