Last year, the Editorial Board decided to produce a special millennium issue for this year to provide an opportunity for global reflection of the many mental health issues we face at the turn of the century. The main themes of this issue were not specifically prescribed. Instead, contributors from different fields were invited to write on global issues that were closest to their own concerns. This has become an important feature of this issue. Although each of the authors and their teams have made notable contributions in their respective areas and have already published large numbers of articles and books, we believe we have a more distilled expression of their perspectives in this issue.

Professor Arie and Dr Mak have written about their visions for service development. Professor Arie has covered the global consideration of future changes in health care service delivery in general, using examples from his own field of 'health care for the elderly'. Dr Mak has focused on the potential for development in rehabilitation psychiatry. Dr He and Drs Zhang have provided us with insights from precious data obtained from an impressive large-scale 10-year prospective community study on dementia in Shanghai. …
