Readers of our Journal may remember that we focused on child and adolescent psychiatry back in 1993. In the current issue, Dr Kelly YC Lai provides an overview of primary prevention in child and adolescent psychiatry, covering the various risks and protective factors, followed by the description of a variety of primary prevention programmes targeted at different stages of development. (1) A systematic application and evaluation of such interventions in Hong Kong is called for.

Extrapyramidal symptoms caused by anti-psychotic medications have been a frequent therapeutic challenge to psychiatrists worldwide. The development of effective drugs with fewer side-effects is much desired. Dr D Basu et al. retrospectively investigated the occurrence of extrapyramidal symptoms associated with the use of an atypical anti-psychotic medication among their patients, and came up with findings worthy of our attention. (2)

Dr SN Chiu et al. found a long treatment delay for patients seen by a community psychiatric team. (3) This is significant since it prolongs suffering for patients, carers, and the community. Future research on the cost-effectiveness of the community psychiatric team with use of appropriate outcome indicators is called for. Similarly, cost-effectiveness is a question that must be addressed in assessing new medications. Dr KY Mak et al. examine the effects of an atypical anti-psychotic medication (coincidentally the same drug investigated by Dr D Basu et al. (2)) on negative symptoms of schizophrenia. (4) Despite its methodological limitations, it represents an initial step for similar studies in Hong Kong.
