'As Good as It's Going to Get'. Bad News Conversations in Neurology: Challenges for Occupational Therapists

Background: This paper presents the findings of an exploratory study investigating the experiences of occupational therapists when having bad news conversations with disabled people regarding likely levels of long-term neurological disability This area of study has had very little research to date.

Objectives: This study sought to answer the question, 'What are the experiences of occupational therapists when having bad news conversations with disabled people regarding likely levels of long-term neurological disability?'

Design: The study design was a retrospective qualitative method within a phenomenological methodology and used semi-structured interviews to collect data.

Settings: Ethical approval was granted by the Faculty Research Ethics Committee at St George's Medical School.

Participants: Convenience sampling was used to identify occupational therapists working with neurologically impaired people of 16 years and over.

Methods: Each participant was interviewed using a topic guide at a time and place of their choosing.

Results: Ten interviews were conducted. Interviewees predominantly had bad news conversations around the time of discharge and there was a functional rather than an impairment focus. Four approaches to these conversations emerged. …
