Dear Editor,

This is just a brief note to convey a few comments about the exceptional articles published in the last issue of Frontiers (Winter 2005, "The Electronic Health Record").

Following my administrative residency in June 1966, I accepted a position with the Laboratory of Computer Science at Massachusetts General Hospital as research associate in hospital administration and later as the assistant director of the laboratory before beginning a more conventional career in hospital management. (The laboratory was then the largest federally funded hospital computer project in the country.) I vividly recall the disbelief provoked in many circles by a 1967 Hospitals article in which a SUNY faculty member wrote that total hospital information systems were at least ten years away. Except for those of us immersed in developing and evaluating computer applications at the time, people considered it heresy to suggest such systems were not going to be fully functional within a few years. Almost 40 years later, too many people still fail to recognize that our capacity to develop new technology has not been matched by a comparable ability to identify and address the organizational and other obstacles impeding cost-effective implementation. …
