We in healthcare find ourselves in a very interesting time. The US Supreme Court recently upheld most features of the Affordable Care Act (ACA), and the country is in the final weeks of a presidential campaign. The outcome ofthat race could set the stage for expansion of the incumbent candidate's landmark and signature healthcare legislation, or it could lead to the ACA's repeal and replacement by his challenger. This time of transition, of course, places considerable stress on those who must navigate the new healthcare environment, which is littered with significant question marks. Certainly, although challenging and difficult, the work of the healthcare executive is never dull. This issue of the journal offers insight, commentary, and evidence that can assist us as we find our way at this remarkable juncture in healthcare.

Our interview is with Donald P. Fesko, OD, FACHE, CEO of Community Hospital in Munster, Indiana, and the 2012 recipient of ACHE's Robert S. Hudgens Memorial Award for Young Healthcare Executive of the Year. Dr. Fesko describes his career transition from practicing optometrist to healthcare executive, offers advice for engaging and motivating the medical staff, and discusses the CEO's role in building a successful leadership team.
