An Innovative Program to Coordinate Interdisciplinary and Interagency Services for Children with Special Healthcare Needs

Children and youth with special health care needs (CYSHCN) and their families often require multiple services from multiple providers to meet their needs. The long-standing acknowledgement of the importance of interdisciplinary, interagency coordination of services for CYSHCN has stimulated a variety of improvement recommendations which historically have employed one-dimensional, "topdown" approaches. These approaches tend to have limited results because the complex and frequently changing requirements of families and CYSHCN necessitate the ability for the complex systems at a local level to form effective coordinating systems across agencies and sectors.

In order to address the challenges with a process that addresses the complexity of the local systems, I will present our approach to the problem from the perspective of complex adaptive systems. This approach requires focusing at the local level with a program that: 1) allows for the organizational development to be sensitive to the unique aspects of each community particularly with regard to existing community relationships, 2) facilitates and adapts the process through a navigator/coordinator not tied to a specific local agency, 3) includes a communication structure between local community programs and agencies and higher government level agencies and 4) provides a motivational system at the local level to help to encourage ongoing activities. …
