Association of Shared Decision Making to Knowledge and Treatment Satisfaction in Dental Patients

Introduction

Research has highlighted shared decision making (SDM) as a method to facilitate a strong clinician patient relationship (1,2). SDM is an approach in which the clinician and patient go through all phases of the decision making process together (1). When SDM is employed, information is provided about the health problem, the possible treatment options and consequences of the treatments are discussed, and together the clinician and patient consider how these fit with the patient's preferred outcomes (1). After all the options are considered, a decision is made based on mutual agreement (3).

The concept of SDM has been thoroughly explored in the medical literature with many benefits reported (2,4-12). Such benefits include improvements in; treatment adherence, patient knowledge, quality of life, satisfaction, general health and wellbeing. It is however evident that little research has examined SDM in the dental encounter (13). With many benefits reported in clinical medicine, it is proposed that dental patients would benefit greatly if SDM was implemented in the dental encounter. This study subsequently seeks to examine SDM in the dental encounter. More specifically, this study seeks to examine the use of SDM in patients who have received treatment for missing teeth and determine if SDM leads to increased knowledge and satisfaction.

This study will provide evidence of the benefits obtained when SDM is employed in dentistry, specifically in patients seeking treatment for missing teeth. It is hoped that this study will demonstrate that similar benefits can be obtained when SDM is used in the dental setting as reported in the medical setting. This study will demonstrate that involving patients in the decision making process is critical in achieving successful clinical outcomes, with particular emphasis on increased dental knowledge and treatment satisfaction. This study is particularly important, as the use of SDM in dentistry has not yet been fully explored.

While this study seeks to demonstrate the importance of SDM in the dental encounter, it is recognized that multiple factors are at play. Mediating factors are expected to influence the relationship between SDM and dental knowledge and SDM and treatment satisfaction. These factors include: procedural factors, pre-treatment masticatory functioning, pre-treatment self-esteem and oral health beliefs/attitudes. These factors will be included in the current study. The aim of this study is to examine SDM in patients who have received dental treatment for missing teeth and to determine if SDM leads to increased dental knowledge and treatment satisfaction. This study also aims to identify factors that mediate the relationship between SDM and dental knowledge and SDM and treatment satisfaction. Specifically, the following objectives of the study will be examined: 1) To determine participants level of knowledge of dental treatment options via the implementation of the SDM questionnaire, 2) To determine participants level of post treatment satisfaction via the implementation of the SDM questionnaire, 3) To compare the SDM group and the comparison group on levels of knowledge of dental treatment options and post treatment satisfaction using linear regression and 4) To explore the mediating effects of procedural factors, oral health beliefs/attitudes, pre-treatment masticatory functioning, and pre-treatment self-esteem on the relationship between SDM and dental knowledge and SDM and satisfaction using linear regression.

The hypotheses of the study

* Patients who have experienced SDM have a higher level of knowledge of dental treatment options.

* Patients who have experienced SDM do not have a higher level of knowledge of dental treatment options.

* Patients who have experienced SDM have a greater sense of post treatment satisfaction.

* Patients who have experienced SDM do not have a greater sense of post treatment satisfaction. …
