This book is a compilation of various strategies on "how to" perform community outreach. The authors developed a conceptual framework whereby a team of nurses, trained community residents, and university-based support staff would work together to bridge the gap between the community, the health care system, and the university. This framework, based on the primary health care model, is applied in a series of projects in the urban communities of Chicago. Each project is thoroughly evaluated in a critical analysis accompanied by tangible ideas for future solutions.

The model, or conceptual framework, is applied to several community projects with different emphases. Examples involve Hispanic immigrant women, health literacy in the inner city, AIDS prevention, school health, the Chicago Health Corps, and the Austin Community Wellness Initiative. Results are presented from the unique perspectives of the individuals involved and with regard to their impact on the long-term health of the community. …
