Pharmacists involved in the management of diabetes patients may want to check out a new software tool that not only automates the planning and delivery of care but also tracks and analyzes clinical results on an ongoing basis.

The Delphi Diabetes Registry software package automates the planning and delivery of patient care; according to Delphi Health Systems, a Windsor, Conn., software firm focused on disease state quality assessment and decision support programs. The program also lets users monitor and analyze clinical results on an ongoing basis, and the risk-stratification function helps users target diabetes patients most in need of intervention.

Using algorithms that automate the creation and implementation of standards-based guidelines, the user selects the measures, parameters, and thresholds to be utilized to collect, monitor, and report data and results for each patient. The program creates and maintains a diabetes medical record for each patient. Patient communications are automated in multiple languages, and the program includes reminder letters about upcoming exams and test results.

Therapy plans document the use of medications and any medication changes. The Diabetes Registry also tracks the use of nondiabetes medications, nutrition, and exercise programs, as well as the use of blood glucose meters. …
