A DAMNING new report out today accuses North social and probation workers of failing a Durham teenager who committed suicide while in custody.

Gareth Price, 16, from Ferryhill, was found hanging at Lancaster Farms Young Offender Institution on January 19, 2005, the day before he was to be sentenced for rape. He was taken to hospital but died the next day.

Durham Local Safeguarding Children Board, which published its serious case review today, said lessons could be learned by all agencies involved in safeguarding vulnerable young people in custody.

LSCB chair, Gail Hopper, said the Board offered its sincere condolences to the teenager's parents, family, friends and all who knew him.

The serious case review started shortly after the teenager's death and aimed to find out if organisations and professionals working to protect young people from harm could learn from the tragedy. The review could not be published until after a coroner's inquest which ended in February this year. …
