SEVERAL fire officers could face disciplinary action after the death of Stephen Hunt.

Greater Manchester Fire Service has launched an internal investigation into the tragedy.

It is likely to be complete before the end of the year.

Jurors at Stephen's inquest found that a lack of communication and information at handovers contributed to his death, as well as misinterpretation of instructions; a breakdown in radio communication; the internal layout of the building; inadequate fire risk assessment, and acts of vandalism and criminal damage.

In a joint statement submitted to the inquest by the brigade and the Fire Brigades Union, nine recommendations are made in the wake of Stephen's death.

They asked that Senior Coroner Nigel Meadows include them in any recommendations he makes to take action to prevent future deaths. …
