In response to the May 8 letter "Medicaid Loopholes for Long- Term Care Are Wrong":

When discussing taxes, can we please take the word "loopholes" out of the lexicon? When taxpayers deduct mortgage interest, real estate taxes and charitable contributions, are they taking advantage of "loopholes" or just doing what the law permits?

Regarding Medicaid, as a country, we need serious discussion of what the law should permit. In the broadest sense, there are two situations that need to be addressed.

The first discussion should address the scenario when you have a community spouse (the spouse living at home) and an institutional spouse (the spouse living in the nursing home). Without impoverishing the community spouse, how much of the assets of both spouses should be spent-down before Medicaid pays the cost of care? …
