EDMONTON. x-13s. A report suggests Alberta policy failures were a factor in the death of a mental health worker by a client who had a long history of unpredictable, violent behaviour. Valerie Wolski was alone with Terrence Saddleback in 2011 when she was strangled in a Camrose care home. Premier Rachel Notlely says the province has improved safety standards since Wolski's death.

TAG: Judge Bart Rosborough recommends that workers should not be assigned to care for people they can't physically manage. A judge made a similar recommendation in a 2008 fatality inquiry report into the death of a woman worker at a Lethbridge care home.
