Dana Lynn Maxwell, a mother of three whose struggles with heroin were described in the 2017 Pittsburgh Post-Gazette special report "Riding OD Road," died Saturday, according to the Allegheny County medical examiner's office.

There was no immediate determination of the cause of death of Ms. Maxwell, 36, of Carrick, who died at her home on Amanda Avenue.

Ms. Maxwell survived an overdose in September 2017 and spoke afterward of how her addiction had affected her life.

"I don't want my addiction to take me to a place where I'm going to end up in jail, away from my kids," she said. "I need to have a plan about where I'm going. .I can't leave out the house without a plan."

The father of her children fatally overdosed in 2016, then she went through periods of recovery and relapse. …
