THE FAMILY CLINIC—VIDEOTAPE CONSENT FORM

During our considerable experience in the Family Clinic, the team have found that the use of videotaped recordings is an invaluable aid in our work with families. It is also sometimes useful in our training programme.

Please read the following paragraphs and, if you are in agreement, sign where indicated.

1. We consent to videotapes being made of these sessions and to these tapes being used to aid the team’s work with our family.
