The purpose of this checklist is to provide guidelines for conducting a proper exercise program. This checklist serves the functions of instruction and monitoring progress. It lists desirable thoughts, emotions, and actions that should be present before, during, and after your exercise sessions. The checklist is intended to help clients form new positive habits that accompany their exercise routines whether they exercise at a fitness facility or at home. Rate each item ranging from 1 (not at all like me) to 5 (very much like me). Please be honest because you want to see improvement over time. Higher numbers are always more desirable than lower numbers, however, it is possible some items are unrelated to your preferences and will never change. Your goal should be to reach higher scores (4–5) over time. Try to complete this form regularly, preferably once per week in the beginning, or as often as you wish.
