D.O.B: 22 May 1971 NOTE: the above-named was discharged from the proceedings and may no longer have a connection with the address listed.

I intend to pay within four months from 8th April 2015 (being the last day for proving) the first and final dividend of 0.22 p/PS. Creditors who have not yet proved their debts must do so by 8th April 2015 (being last day of proving) otherwise they will be excluded from the dividend. The required proof of debt form is available on the Insolvency Service website (www.bis.gov.uk/insolvency, select "Forms" and then form 6.37).
